U Got This! is more than a wellness program—it’s a movement.
We’re here to help members of the African American community take charge of their health, build confidence, and gain the tools to manage or prevent diabetes through real education, modern technology, and personal support. Our mission is simple: to close the health gap by combining innovation, connection, and care that meets people where they are.

For too long, diabetes and related chronic illnesses have impacted our community at higher rates—while access to quality resources and culturally aware support has remained low. We’re changing that story.

U Got This! was built to bring awareness, access, and action together.
Through education, wellness coaching, and cutting-edge tools like continuous glucose monitoring, we help people understand their bodies, make informed choices, and see progress in real time.

Say Goodbye to Finger Pricks!

CGM provides real-time glucose readings throughout the day and night. Small, wearable CGM devices replace the finger sticks of traditional blood glucose testing. Most CGM sensors are tiny self-applied devices that sit partially beneath the skin. They come with an inserter and have built-in adhesive to help them stick to your skin. Easily check your glucose levels with a painless 3,5 one-second scan instead of a finger stick.

The sensor measures glucose levels at regular intervals and stores the readings. A handheld reader allows you to scan the glucose sensor to collect your information. You may be able to use a smartphone app so that you don’t have to carry around an extra device. CGM gives you a view of your glucose levels over time; it can help you keep track of past glucose levels and provide insights into future trends.

Rather than taking glucose readings from your blood, CGM sensor readings are taken from interstitial fluid, a thin layer of fluid that surrounds the tissue cells below your skin. As carbohydrates are digested, glucose enters your bloodstream before it is absorbed into the interstitial fluid. Now with optional, real-time glucose alarms that notify you if you go too low or too high.

The FreeStyle Libre 3 Plus sensor lets you see your glucose levels in real time so you can make more confident choices and lower your A1C.

Proven to be accurate with strong, reliable connectivity. Optional alarms let you know the minute your glucose is too high or too low.

Who Qualifies?

18+ Years of Age, Columbus, OH Resident.

You must be above the age of 18 to participate in the pilot program. You must also reside in Franklin County.

Meets Income Requirements.

You must be Medicaid eligible in order to qualify for the pilot. If you are not sure if you are Medicaid eligible, please contact your doctor or contact us.

Non-CGM User.

Participants cannot currently be using a CGM device, or have had used one in the last 90 days.

No. This program has no cost to participants. If you qualify, you will receive a CGM device, a wellness coach, and free access to online support.

You will need to consult your primary care doctor to receive a prescription for the CGM device.
